New Treatment for DVT and Tips on How to Avoid One

The National Institute for Health and Clinical Excellence (NICE) recently issued its final appraisal determination on the use of Rivaroxaban (also known as Xarelto) as an alternative to Warfarin for the treatment of Deep Vein Thrombosis (DVT) and the prevention of recurrent DVT.
Up until now, Warfarin has been the standard treatment, but it requires regular monitoring by blood tests (INR), dosage adjustments and people taking Warfarin need to careful about their diet, because Warfarin can interact with certain foods. Rivaroxaban does not require regular monitoring and it represents a potential benefit to people who have had a DVT.
Rivaroxaban is recommended as an option for treating DVT and preventing recurrent DVT and Pulmonary Embolism Thromboembolism (PET) after a diagnosis of acute DVT in adults.
In relation to DVT, prevention is better than cure so here are my tips for DVT prevention.
Tips for avoiding a DVT
  • Maintain an active lifestyle and exercise regularly -- daily, if possible. Walking, swimming, and bicycling are all great activities.
  • Manage your weight with exercise as well as by eating a healthy diet.
  • If you smoke, quit!! Nicotine therapy (in patches, gums, or sprays) and support groups can make this much easier to do.
  • Get your blood pressure checked regularly; take steps to lower it, if necessary.
  • Report any family or personal history of blood-clotting problems to your doctor.
  • Discuss alternatives to the contraceptive pill or hormone-replacement therapy with your doctor.
  • If you are on a plane for more than 4 hours, either walk or do leg stretches in your seat and also stay well-hydrated and avoid alcohol consumption.
  • If you are going into hospital, make sure you have a "risk assessment" and that you are considered for anti-thrombosis socks and blood thinning injections.
Clinical-effectiveness data from a trial comparing Rivaroxaban with Enoxaparin and a vitamin K antagonist in people with venous DVT and PET was applied in the NICE decision-making as was Enoxaparin and a vitamin K antagonist as the key comparator. The NICE Committee concluded that the difference in dosage did not appear to be clinically significant and was satisfied that the comparators used in the trial represented routine and best practice in the NHS.

Types of DNA Analysis

A process of conducting a study about genetic sequences, DNA analysis is being used by medical personnel and law enforcement agencies to identify a particular species or person. It can even be used to determine specific diseases and cancers. At one point, this process becomes very limited. However, DNA is now analyzed using numerous processes.
The forensic DNA analysis comes in three different types: Y-Chromosome, STR or Short Tandem Repeats (uses PCR or Polymerase Chain Reaction), and mtDNA or Mitochondrial DNA.
Y-Chromosome Analysis
The Y chromosome is very useful in forensic applications as it helps identify the suspects. This chromosome can only be found in males and can only be passed down from a father to a son. Hence, analyzing this chromosome separates the female DNA and identifies familial relationships. Another advantage it offers is its ability to limit the pool of potential suspects even if there are numerous male contributors found in the biological sample. With the latest advancements, the process of extraction that is used to separate the vaginal cells from the semen prior to the DNA analysis becomes unnecessary.
Short Tandem Repeat or STR Analysis
Short Tandem Repeat or STR analysis examines the particular areas or loci in the nuclear or normal DNA. Each person comes with a distinct region of DNA. This process improves the difference of one DNA profile from the others. In cases of identical twins, the chances of two people sharing similar 15-loci profile is still one in a 1,000,000,000,000,000,000,000,000,000.
The FBI (Federal Bureau of Investigation) set a standard of 13 STR loci for the STR analysis and for DNA databases. This is to ensure that all forensic labs will have the ability to share and use the same forensic information. The CODIS database for unsolved cases creates profiles using the STR process, according to the 13 STR loci guideline set by the FBI.
Mitochondrial DNA Analysis
The mtDNA or Mitochondrial DNA analysis is used to examine samples that cannot be analyzed through an STR analysis due to non-availability of a nucleic DNA material. Unlike the STR analysis that uses the cell's nucleic DNA, the mtDNA extracts DNA from old sample evidence such as bones, teeth, and hairs that no longer have any nucleic DNA material. Though the STR analysis can also be used in creating DNA profiles from an old evidence, but the sample materials like semen or blood are sometimes destroyed in the PCR testing while generating an accurate profile. Nonetheless, the Mitochondrial DNA analysis can be used to examine an old sample material like a hair shaft and eventually helps solve the case. Off-Season Strength and Conditioning Programming

As winter winds down and turns to spring (hopefully), it's that time of year again for off season training to begin for a lot of sports. I figured now would be a good time to throw a few things out there to help you better prepare your athletes for next season.
1. While a lot of people think that their sport is "different" and has a totally unique way in which to train for it, they are generally misinformed. Sports need a lot of qualities developed in various quantities, but your job as a strength coach is to improve their general physical preparedness. That is, get them stronger, more powerful, and more resistant to injury. The qualities they develop in the weight room will be used when they practice their sport. Don't get carried away and sucked into the "sports specific" idea. Mimicking sports movements in the weight room won't typically get you very far. In fact, you generally want to do a lot of the opposite of what they do in their sport, in order to get the body more in balance so they are less likely to get hurt down the road.
2. Start planning your off season training program backwards. Start at next year's preseason, and write your phases out backwards. You need to know where you're going if you want to get there as efficiently as possible.
3. Focus on strength and power improvements, but don't disregard conditioning. There's no reason to be out of shape just because it's the offseason, but you don't have to have them in peak game shape either. Progress their conditioning as you would progress their other qualities.
4. If the athletes need to add mass, then they have to eat more. Just loading on a ton of extra volume without eating more will just beat them up, not make them better. If you put an emphasis on getting stronger on the big compound movements and they eat right, the mass will come. Guys that move big weights look like it for a reason.
These are just a few things that I had bouncing around in my head, and there are obviously other things to take into consideration when planning your off season strength and conditioning programs. If you train more than one sports team, remember, the programs for the different sports should look a lot more alike than different-as in 90-95% alike. This is a crucial time of the year that will help prepare your athletes for the rigors of their sport next season, and a lot of injuries that occur during the season are able to be reduced by proper off season training. Plan correctly and give your teams a head start on the competition.
